A survey of almost 6,000 smokers in Britain found those who vaped Electronic Cigerettes were more likely to quit smoking tobacco

The e-cigarette industry got some good news this week: A survey of almost 6,000 smokers in Britain trying to quit found that those who used electronic cigarettes were more likely to stop using smoking tobacco than those who used over-the-counter quitting aids or had no help at all.

The study, accepted by the journal Addiction and published online Tuesday, comes as U.S. regulators weigh new rules for nicotine vaporizers. The e-cig industry and public health officials are battling over whether the devices should be treated as less-harmful cigarettes that help smokers give up tobacco or as a gateway that will lead adolescents to a deadly habit.

Investigators from a cancer research center at the University College London conducted the study, which was partially funded with government and drug industry grants. None of the authors reported financial ties to e-cigarette companies, which adds to the report’s credibility.

The latest analysis drew on surveys of British households from 2009 to 2014 and counted smokers who had tried to quit within the past 12 months. A majority used no help, about one-third tried over-the-counter aids, such as nicotine patches or gums, and 8 percent used e-cigarettes. The study excluded people who used prescription medicines or counseling to quit smoking, as well as those who used both e-cigs and nicotine replacement. E-cig users were more likely to have stopped smoking tobacco by the time of the survey than either of the other groups.

While surely welcome news for fans of vaping, these survey results are not the kind of evidence that proves e-cigarettes are effective quitting aids and would let the industry market them as such. For that, regulators want to see rigorous, randomized control trials that compare e-cigs with placebo treatments. E-cigarettes are also an evolving technology, with varying formulas and designs that may have different effects on users. And the research doesn’t address whether smokers relapse more when using e-cigs than other quitting methods.
Scientists also haven’t established what risks e-cigarettes might pose to long-term users. It’s impossible to know how these products will affect people over decades, because the products are less than 10 years old.
Other risks associated with e-cig use: They occasionally explode, and children or pets may accidentally consume the poisonous liquid if it’s left unsecured.

While vaping is widely believed to be safer than inhaling tobacco smoke, it’s “not safer than just breathing clean air,” as Dr. Richard Hurt, former director of the Mayo Clinic’s Nicotine Dependence Center, said in a recent interview. 

Other research in the U.S. suggests that smokers’ perception that e-cigarettes are less harmful is declining.

Still, many people report anecdotally that e-cigs help them trade a habit known to be deadly for one presumed to be less dangerous, even when other approaches failed. To the extent that e-cigs help people quit inhaling tobacco, the study says, “e-cigarettes may substantially improve public health because of their widespread appeal and the huge health gains associated with stopping smoking.”

Electronic cigarettes can change gene expression in a similar way to tobacco, according to one of the first studies to investigate the biological effects of the devices.
Presented at the American Association for Cancer Research annual meeting on 6 April in San Diego, California, the research looked at human bronchial cells that contained some mutations found in smokers at risk of lung cancer. The cells were immortalized, grown in culture medium that had been exposed to e-cigarette vapor and their gene expression profiled.
The researchers found that the cells grown in medium exposed to the vapor of e-cigarettes showed a similar pattern of gene expression to those grown in a medium exposed to tobacco smoke (S. J. Park et al. Clin. Cancer Res. 20, B16; 2014).

The changes are not identical, says study researcher Avrum Spira, who works on genomics and lung cancer at Boston University in Massachusetts. But “there are some striking similarities”, he says. The team is now evaluating whether the alterations mean that cells behave more like cancer cells in culture.

The work is at a very early stage and therefore cannot establish that e-cigarettes can cause cancer in vitro, let alone in vivo. “They may be safer [than tobacco], but our preliminary studies suggest that they may not be benign,” says Spira.

E-cigarettes are extremely controversial. Because they vaporize liquid containing nicotine, rather than burning tobacco, some researchers believe that the devices could greatly reduce the damage done to health by smoking; others, however, argue that they are simply ‘renormalizing’ smoking.

As far as I can see, e-cig does have bad influence on people's body but its appearance at least save many second-hand smoker. If you smoke, why not choose e-cig for good of you and others.

Virtually all newcomers to electronic cigarettes are smokers [1], so it is natural for them to try to use an e-cigarette in the same way as a tobacco cigarette, especially as the first model chosen may be a replica of a cigarette.
Unfortunately this doesn't work well, as the two things may look similar but are functionally very different. One is a paper tube where the smoke from burning chopped-up vegetable matter is inhaled; the other is a steel tube producing a water-based vapor from a liquid that is vaporized by a battery and heater element - essentially, a low-temperature steam. One contains tobacco that is set on fire at high temperature; the other has no tobacco and generates mist using a battery. There aren't really any valid comparisons between the two.

The most accurate term for an 'e-cigarette' is probably an EV, electronic (or electric) vaporiser. The vast majority of EVs do not look anything like a cigarette, although the sales figures are opposite to this: minis that look like cigarettes do have huge sales volume as they are by far the most popular with smokers trying out the concept. In addition there is no functional similarity e-cigarette' as marketing terms since most EVs don't look anything like a cigarette and none work anything like a cigarette. However, hardly any smokers would switch if the name EV or something similar like PV or vaporiser was used exclusively - so 'e-cigarette' works very well indeed to get smokers interested. This tends to obscure the fact that the two things are different and are used differently. Mini ecigs, especially, need a different technique in order to derive maximum benefit with minimum problems.

Usage
If an ecig is used like a cigarette, it doesn't work, and there might be other issues too, such as a sore throat or lungs. This is generally applicable to a new ecig user with a mini ecig although it may not apply in other circumstances (expert users, with better hardware, make their own rules - this info is mainly for newcomers using minis).

An ecig doesn't contain any tobacco, there is no combustion, there is no ignition, there is no smoke; the process involves generation of low-temperature 'steam' from within a saturated atmosphere in an electrical device - not the burning of dry plant material. Multiple clinical trials have demonstrated that if an ecig is used in the same way as a tobacco cigarette, by beginners using a mini ecig, either zero or very little nicotine can be measured in the user's bloodstream.

So now that we know it must be used differently, the question is: how? The short answer is that the only similarity is that both are placed in the mouth. After that, everything else is different, when talking specifically about optimal use for new users using low-power hardware.

There are a multitude of myths connected with vapor: how it should be inhaled, what vapor is comprised of, and so on. Some of them are below. This article is probably best read along with the one at: Inhalation technique with an e-cigarette

1. "No one uses a direct lung inhale with a cigarette"; or, "No one uses a mouth inhale with a cigarette".


2. "People mainly use a mouth inhale with ecigs", or "People mainly use a direct lung inhale with ecigs".

Wrong. People do both. Who does what may relate to whether or not they are a beginner, or using a mini ecig, or if they did one or the other when smoking.

Beginners should always start with a mouth inhale when switching to ecigs, as there is far less chance of


lung irritation. A variety of other methods are used by experienced vapers according to the hardware or their preference.
3. "Ecigs irritate the lungs."

Wrong. If a beginner uses a direct lung inhale, then that is an almost certain recipe for lung irritation. Beginners should use the mouth-first-lung-second inhalation method only. Experts can do as they like, because they will know how to avoid lung irritation.

4. "Ecig vapour is mostly PG/VG and there isn't much water, because there is no water in e-liquid."

Some glycerine-base refills contain over 10% distilled water in the finished e-liquid (up to 20% is added to a glycerine base to reduce its viscosity and make it suitable for vaping).


The average amount of water found in vapour tests of mini ecigs is 66%, and the PG/VG total content is around 5 or 6%. This is explained by the fact that normal exhalation contains a significant quantity of water vapour (therefore - rather obviously - there must be some in exhaled ecig vapour; so a test that shows none at all is clearly faulty). We don't know anything about vapour produced by more efficient hardware, since at Q1 2014 no one has published an analysis of it.
